How to Submit A Preventive Reimbursement Request
Our members love getting reimbursed for eligible preventive expenses, and we make it easy!
First, find the preventive reimbursement form. There are two ways to find it:
- Website: You can also fill out the online preventive reimbursement form using this link: https://healthaccesssolutions.com/preventive
- Text: Text the number “3” or the word “preventive” to (239) 299-0333. You will receive a text back with a direct link to the online preventive reimbursement form.
Note: Having trouble with the text option? This only works if your text is sent from the cell phone number associated with your account. Contact HAB Member Support at (800) 606-1135 to confirm that we have the right cell phone number on file.
Then, complete the preventive reimbursement form. Fill out all required fields, then snap a picture of your itemized receipt. You can submit up to 5 images per request, including ones from different providers and/or different dates of service. Each itemized receipt should include:
- Provider name
- Patient name
- Date of service
- Breakdown of the services rendered
- Amount charged for each service
Once your preventive reimbursement form is complete, click submit. Your request will be processed within 10-business days. It’s really that simple!
Eligible Preventive Reimbursement Services
Which services are eligible for preventive reimbursement?
- Basic wellness visits, including annual gynecological exams and mammograms
- Some vaccinations, like the flu or pneumonia
- Lab work, such as Complete Blood Count (CBC)
- Vision exams and dental cleanings
- Mental health counseling or therapy
- Gym memberships and personal trainers
- And more!
