Your plan is part of our Any DPC program, which means you can select any direct primary care provider you would like and submit the bill each month to be reimbursed just like with your preventive care services. A direct primary care provider delivers services offered by physicians and nurse practitioners with a simple flat fee membership. Find your maximum allowable amount here.

Keep Your Primary Care Provider

If you do not want a subscription with a direct primary care provider, you can keep your current primary care provider with no network limits, and get reimbursed for multiple Annual Wellness visits per year. Find your maximum allowable amount by logging into your Member Dashboard here.
