Your Health Access Benefit, and specifically the Preventive Care component, is designed to keep you and your family healthy. Starting May 1, 2023, you can experience quicker turnaround times and responsiveness when filing a reimbursement request.
Here’s How It Works
We make reimbursements simple for our members, and this new update will deliver even more simplicity and speediness, allowing our team to process your reimbursement request in less time.
First, visit the provider of your choice for Preventive Care as a self-pay patient.
Next, fill out the reimbursement request form. You’ll need the following information:
- New: The date of service rendered
- New: What category the reimbursement falls under (Basic Wellness and Labs, Physical Health, Nutritional Health, etc.)
- New: The amount you are requesting for reimbursement
- A photo of your itemized receipt for reimbursement
